A coded clinical condition or diagnosis attached to a patient — and optionally to a specific encounter — sourced from the provider's clinical record.

Fields

Consumption notes

• Match on code.system + code.code (see Coded concepts); treat code.display as convenience text only, not a matching key.
• encounterRef may be absent — not every condition is tied to a specific visit.
• clinicalStatus is optional; its absence does not mean unknown — it means the source system didn't report a clinical status for this record. Treat any value your integration doesn't recognize (including unknown) defensively.
• The resolution-family values inactive, remission, and resolved all indicate the condition is no longer active, with different clinical nuance between them — surface the raw value to your users rather than collapsing them to a single "inactive" state.
• patientRef/encounterRef are source references; correlate them to the canonical patient and encounter records whose identifiers contain the matching business identifier.
